We recommend all participants to be covered by medical health insurance before entering the
tournament.
I understand that any division may be cancelled for lack of participation. I, the undersigned, do
voluntarily submit my application for attendance and participation in the Vallejo Goodwill
tournament. I fully recognize and acknowledge that this competition frequently involves physical
contact despite the necessary safety precautions and rules, and I enter such competition at my
own risk. Therefore, I assume full and complete responsibility for any and all damages, injuries, or
losses that I may sustain or incur, If any, while attending or participating in the competition. I
release from all obligation and liability for injury to myself - ESKCOA, promoters, sponsors,
operators, officials, Philippine Cultural Committee or other competitors, individually or otherwise. I
realize and acknowledge that I am solely responsible for any medical attention or treatment I may
receive will only be of the first aid type at the tournament site. I declare that I am free from any
physical disorders or illness which might prohibit competition. I agree to abide by the rules of the
tournament and the final decisions of the officials. I certify that all the information on this
application is correct and accurate to the best of my knowledge.
